Job Description
The REACH Project, Inc is a non-profit organization that provides low-threshold, harm reduction-oriented healthcare to all who typically face stigma in the current healthcare system.
Position Goals and Role:
● This is a full-time position at REACH Medical, PLLC.
● The LPN role ensures that all patients receive the highest quality health care in a harm reduction setting.
● REACH aims to consistently offer a productive, professional, friendly, stigma-free work environment for staff and clinical providers.
General duties:
● Responsible to ensure patients are scheduled for the appropriate visit type and seeing all patients prior to their provider
● Obtain vitals, and proper patient history, including managing the nursing phone line for both Primary Care and Medication Management.
● Other duties include, but are not limited to:
A typical schedule will start at 9:00 am some days and extend into evenings on others.
Qualifications include:
● Currently valid NYS professional license
● Experience with electronic medical record system and related workflow processes
● Schedule flexibility
● Strong interpersonal, with a commitment to provide respectful, culturally-competent communication, skills that match the needs of our target population(s)
● Ability to demonstrate a commitment to providing care to underserved populations consistent with REACH Medical’s reduction-based, trauma-informed, low threshold care philosophy.
