Job Description: Clinic Nurse-LPN

Reports to: PMHNP and Practice Administrator

Job Summary: Work as a team member with clinicians, the Medical Support Staff/Nursing team, and office staff to provide patient care following practice protocols developed by the management team. This includes communication with patients and their families for crisis and emergency phone triage, in addition to guidance and assistance regarding medication issues, overall health concerns, health/wellness education, and medication refills determined by the primary prescribing clinician.

Primary Job Responsibilities:

• Gather information and triage phone calls from patients. Inform and consult with the treating clinician as needed. Document interactions with patients in the patient chart.
• Provide phone advice and education (under guidance from primary clinician) to patients regarding questions they may have about the use of medications, medication side effects, and other health concerns.
• Complete prescription refill requests: clarify with pharmacies, verify correct medication and doses, monitor frequency of medication refills, document all refills and medication changes in the patient chart.
• Coordinate with the Front Office Staff to provide patients with appointment reminders to comply with clinician’s orders.
• Communicate with insurance companies to complete medication Prior Authorizations and Appeals .
• Communicate with patient caregivers regarding medication doses and side effects and changes in therapy. Provide Doctor’s Orders Letters and Medication Change Letters to care facilities, as needed.
• Complete the following forms and letters, as needed: Medication administration letters for school/camp, IEP forms, Emotional Support Animal letters, Drug Prior Authorization and Denial Appeal letters, Family Medical Leave Act forms.
• Communicate with school nurses as needed to ensure correct administration of medications at school.
• Assist patients and clinicians with referrals for specialized testing such as sleep studies, EEG, ECG, neurology consultations and other tests/consultations as ordered by clinicians.
• Order patient sample collection kits for pharmacogenomic tests. Document results in the patient chart.
• Labs: Mail/fax lab orders per lab protocol or clinician request. Maintain system to notify patient when labs are due. Review lab results, document labs in the patient’s chart, and route to clinician. Communicate with clinician re: follow up for abnormal results, as needed.
• Provide case coordination for patients receiving clozapine therapy. Coordinate with lab, pharmacy, clinician and patients to ensure that labs are done per Clozapine REM protocol. Enter lab results on clozapine registry.
• Facilitate coordination with primary care physicians through sending a Health Care Coordination Form (HCCF) after the initial evaluation and on a regular basis thereafter.
• Communicate with the other members of the Medical Support Staff to provide seamless care to patients throughout the week.
• Update and maintain patient health records in accordance with HIPAA standards and MMPC Policy and Procedures.
• And monthly medical support staff meetings and monthly all staff meetings.
• Other duties as assigned.

Benefits: Employer sponsored health benefits, 401K with employer match, paid vacation time, and a focus on work-life balance. Starting salary range is $38.00-$40.00/hour. This is a 40 hour a week, in office position.
